What should I bring with me to my appointment?

Be sure to bring your insurance card(s), referral paperwork, copayment, medication list, and all eyeglasses you currently wear (including readers). Please leave valuables, personal items and items that make a lot of noise at home.

I’m a new patient. How early should I arrive for my appointment?

Please arrive 15 minutes before your scheduled appointment so that you can sign in and present your insurance card, referral paperwork and copayment at the front desk. We will have you complete the necessary paperwork. Appointments usually last approximately 30 minutes to 1 hour.

May I bring someone with me to my appointment?

You are welcome to bring a family member or caretaker with you to your appointment if necessary. They are welcome to join you in the exam room or wait in our waiting room while you undergo any necessary tests.

Is your location handicap accessible?

Yes, the building includes wheelchair ramps, handicap-accessible facilities and there is an elevator.

Where can I park?

Please park on the street at one of the metered spaces or park in one of the lots on 150th Street. Parking costs $10 per day.

How do I find the practice?

The closest cross-streets to the practice are Cortland and Morris. The nearest subway stop is Subway 2 & 5 at 149th and 3rd Ave. The practice is located in the Bank of America building on the second floor. Use the front entrance to enter the building. Use the top buzzer to open the door then take the elevator to the 2nd floor.
